Technical Program Dual Credit Student and Parent Informed Consent Form

Your KCTCS College requests your acknowledgment of the realities of life on campus during the COVID-19 pandemic. If you are a dual credit student over 18, please fill in the information blocks, check the student signature box, and sign in the signature box. 

If you are a dual credit student under 18, you and your parent/guardian should agree to this document. Please fill in all the blanks, select both the student and the parent/guardian check boxes, and your parent/guardian should sign in the signature box.

Thanks for taking the time to complete this form. An email response will be sent to the student’s email that they have completed the form. A faculty or staff member on campus may ask to see this email so they know the form has been completed.
